Suggest Treatment For High Blood Pressure

Question: I have high blood pressure problem and my doctor has been prescribing lots of medication for me and
they have not been helping me. I am getting frustrated and need help.

I have gone through your query with diligence and would like you to know that I am here to help. After going through your list of medications, I completely understand your plight. I agree these are indeed too many medications to help control hypertension.

Before I provide you with my opinion, I would like you to understand why there are different class of drugs that can be used to treat hypertension. This is because each group of drugs can have a different effect on controlling blood pressure. This is just information for you sir, and nothing more.

Groups like beta blockers, calcium channel blockers, and angiotension-converting enzyme inhibitors (ACEi) are usually for patients with uncomplicated and easily responsive hypertension. But in some patients (like is your case), patients do not respond well to the simpler anti-hypertensives being prescribed. These require more serious and stronger anti-hypertensives (e.g. direct vasodilators, central and peripheral sympatholytics, etc.). Some of the more stronger ones have been tried in your case (as seen from your list of drugs), which means yours is indeed a difficult case sir. With an unbiased opinion that I carry, I am afraid I understand why your doctor seems to be struggling to control your blood pressure.

In such circumstances, it would be best for you to gain a second opinion from a doctor in the same area as you live who is more experienced in the field, as you will require regular access to this doctor.

In my opinion if I were your doctor, I would go with two drugs maximum, most probably a combination like:

1. Hydrochlorthiazide (25mg)/spirinolactone (25 mg) one tablet to be taken once daily

2. Minoxidil (30-40 mg/day); i.e. 20 mg twice a day

You can choose to take this up with your doctor sir, but it is my sincere request that you be patient with your doctor as yours seems to be a complicated case, and I am sure your doctor is doing his best to help control your blood pressure. Finding the right combination of drugs for each individual with hypertension can indeed be a task.
