Iam 37 female. im very much tensed about type of my life. In 2010 I went to doc after a major anxiety attack. went to all cardi, neuro endo.. the endo diagnozed me with high insulin levels. Iam hypthyroid as well since 14 years. These episodes of anxiety and attakcs are not getting lowered instead getting increased day by day. I have done all MRI's EEG, sTRESS ECHO, eCHO, tmt and everything is fine. I was taking 1000 Glycomet twice since so many years but now this is reduced to 500 mg twice daily by my cardio.
medicaltions which im taking right now is thyronorm 100 mcg, LEVIPSY 1 GRAM once daily, Oxycarb 300 thrice daily, Gapapentin NT, pregablin 75 twice daily. Ivabradine 5mg twice daily to regulate heart beat.
Iam attaching all my reports. Please tell me the followng
1- Why insulin is so high (this is PP report and I actually forgot to nt to take medicine, this is after medicine.)
2- TSH is 0.3 ( I have a very small cyst in noduile of thyroid. FNAC couldnt be done as its very small. ) its going towards hyper side.
3- My BUN ratio is also little disturbed in the report.
Iam very very upset. I excercise a lot. Have extreme control on my diet as well and following diet given by nutritionist. How can I be treated? Iam fatigued all the time. despite doing so uch excercise my weight is not getting decreased.
Please help!
Insulin resistance:
Detailed Answer:
Hello,
I have gone carefully through your concern.
Regarding to high insuline level, it is a common finding at overweight or obese persons (please attach your BMI and abdominal circumference) and is called insuline resistance. Your physician has started metformine and I do agree with him.
To improve it you should loose weight. Physical activity should be accompanied with low carbohydrate diet.
The TSH Level is at the lower normal range. It means that the levothyroxine dose should be lowered slightly ( from 100mcg to 85 mcg daily) in order to keep the TSH at the median ranges. The nodule is too small ( 0.3 cm) but an ultrasound should be done after 3 months.
The BUN level is a bit low, not suggestive for any pathology.
Hemoglobin is 10.6 ( showing anemia but at the levels to be symptomatic). You should check vit B12 and acid folic to find out it's cause, because ferritin is within the normal ranges.
Hope i have answered your queries.
Kind regards,
Dr.Mirjeta

Metformine 1000 mg daily would be the right dose:
Detailed Answer:
Welcome back,
1000 mg of metformine daily is the recommended dose to treat insulin resistance, as long as you are overweight.
A dietitian would probably be of help in reducing the weight, if you have not had any result until now.
ME 12 contains acid folic and VitB12 so it would help to treat the low Hemoglobin level.
