Suggest Treatment For Hyperhidrosis

default
Posted on Fri, 11 Sep 2015
Question: Dear Doctor,
I am nearing 30, male and unmarried. I suffer from hyperhidrosis, excessive sweating. This is the major problem bothering my daily work as i do public speaking every day. This excessive sweating problem happens even when there is little stress i start to feel, though in a cool environment. Very embarrassing. There is tremendous internal heat generated and hot flashes across body (at hands, now lately at foot). I have other problems - abdominal pain (right side) and genitals with scrotum enlargement . I have gastroenteritis and feel stomach bloating. The only bad habit i have is to masturbate atleast once a week and feel some relief. However I did understand this on long run only has toiled down by body and hyperhidrosis has developed only as side-effects of these. Over these years I have consumed Inderal (to relieve my stress) has cooled me down mentally but my sweating is unable to control. Anti-antiperspirants are of no use. Fresh juices is something i take but not an remedy to sweating. Have tried several online-forums , but of no-use.
Every problem I have is embarrassing to speak-out and sincerely hope I get a solution here.
Thanks.

1.Since how long you have this issue.?

Excessive sweating has been for more than 1 year. It has grown worse in the amount of sweating over these years

2.Is there any family history of this condition?

As far as sweating is concerned, no family history. But prostrate problem and thyroid problem are there for my parents respectively.

3.What regions of your body are most affected by excessive sweating-only palms and foot etc or total body?

Face is the most. My underarms is the next. My shirt becomes wet fully.
Over the past 2 to 3 months, my foot has hot-flashes.

4.Did you have any associated past history of any neurological or thyroid illness?

Nothing like that. I feel the habit of masturbation over the years has only influenced neurological weakness and consumption of medicine like NF cure/Shilajit over 6 months prior to excessive sweating problem has induced it more.

Brief Answer:
Possible Secondary GH. Relevant info provided.

Detailed Answer:
Hello Sir,

I understand your concern.

Going by your history you probably seem to have Secondary Generalized hyperhydrosis/GH.

Masturbation is no where implicated in causing hyperhydrosis.
Likewise NF cure and Shilajit are not known to cause hyperhydrosis.

GH is your case could be secondary to long term use on Inderal.

This drug is directly known to cause GH.

Likewise the scrotal enlargement and pain abdomen have to be seriously evaluated since any infection(like abdominal or genital TB ) is known to cause or aggravate GH.

In this context I would like ask you another query--Is there any nocturnal hyperhydrosis?-excessive night time sweating.

To rule out these medical issues I advise you to get an Ultrasound abdomen as Scrotal Ultrasound Doppler .These investigations are mandate in your case.

Additionally I advise you the following blood work up.

1.Serum uric acid levels.
2.PPD test(to screen for tuberculosis).

Gradual tapering of Inderal will help to improve your hyperhydrosis to a large extent. This has to be done after consulting your primary care physician.

Further management is based on the test results.

Meanwhile symptomatic therapy can be used to control excessive sweating in your case.I generally treat my patients a short course with oral tablets of Glycopyrrolate. I suggest you to please see a doctor if his views are same and if can prescribe these drugs for you.

Post your further queries if any.

Thank you.

Dear Doctor,

Thank you very much for your reply.

As per your advise, I will seek to have Ultrasound performed. Feeling quite embarrassed though.

Regarding Is there any nocturnal hyperhydrosis?-excessive night time sweating.,

Doctor, I can have any-time sweating. If Physiologically/psychologically I feel quite stressed, steam of sweat automatically pours down. I do sweat a lot after masturbation. Hence I linked everything to masturbation which has affected me in long run. I do live/sleep in AC room, hence no such noctural hyperhydrosis. But it is guaranteed to switch off fan, the next moment i would start wetting.
Kindly help.

Glycopyrrolate - Robinul is known to cause side-effects doctor? I was prescribed Inderal rather than robinul as it is safe. Kindly advise.

Should i have to check up with urologist? Kindly advise on that also.

Brief Answer:
Urology consultation is important.

Detailed Answer:
Hello Sir,

I understand your concern.

Yes you need to consult a urologist for scrotal enlargement.

Masturbation is not linked to your hyperhydrosis as already mentioned.

An element of autonomic system hyperactivity is also contributing to your hyperhydrosis. This can be corrected by Robinul.

Robinul is relatively free of side effects when used in prescribed doses and duration.Occasionally it can cause dry mouth that can subside on its own.

Inderal is directly involved in causing excessive sweating. How can you use a drug that can directly cause this problem.Better avoid it.

Post your further queries if any,
Thank you.

Brief Answer:
Relevant data suggested.

Detailed Answer:
Hello Sir,

I understand your concern.

The standard dose is 2 mg tablet twice or thrice a day based on the symptoms.

The duration should be at least 4 months.Further requirement is decided by your physician.

Hyperhidrosis due to psychological stimuli can hinder the social life of patients, lower their quality of life, and may even result in psychological problems such as depression .

Also few individuals with autonomic nervous system hyperactivity are known to have untoward effects on heart function in the long term.Hence I recommend you to have a semi annual screening of heart via 2D echo.

1.Cornstarch
This is a simple method that can be employed as and when you feel that you are starting to sweat profusely. Carry a bottle of cornstarch powder, which can be lightly perfumed with a talcum powder as well and apply the cornstarch on the areas that sweat profusely.

2.Tea tree oil.
Tea tree oil is very strong and must be applied on the skin only after diluting with water. Apply a thin coat of tea tree oil on the area like armpits, back of the neck, feet, face, groin area, inner elbows and behind the knee. Allow the oil to dry out and reapply as needed. This oil is known to prevent fungal infections associated with excessive sweating.

3.Baking Soda
Take the powder and dust under your arms and in areas where you sweat. A coat can be left inside the shoes as well. Baking soda can also be mixed with cornstarch to get a more potent drying remedy.

4.Potato Slices
Slices of potato are great for absorbing water and this property of the vegetable can be used for controlling sweat as well. Potato slices are most effective on areas where you can quickly reach like the armpits, neck, inside of the elbows.

5.Foods to avoid include spicy dishes, garlic, onions, processed and high-sugar foods, and foods with a high concentration of hydrogenated oil. These foods stimulate your sweat glands, causing excessive perspiration.

6.Reduce intake of coffee.

7. Avoid hot drinks, alcohol, nicotine ,hot baths.

8.Drink Sage tea and tomato juice.

9.Use antibacterial soaps like Dettol .

10.Drink plenty of water.

Thank you.
