So for that no treatment is needed as your thyroid hormones are within range and TSH not much high.
You have to repeat TSH estimation and free T4 estimation after one month and revert back with that reports.
Your B12 level is in borderline range. If it is less than 150 than deficiency can be considered.
However if weakness and tingling present than B12 injection once a week for six week can be prescribed.It's advisable to investigate with CBC as well to check your hemoglobin level.

There no need of treatment for high TSH.
Your TSH value is not much high. It's a borderline value. Moreover your T4 is within limit.
Non thyroid illness and mild inflammation also can alter TSH value.
So just repeat TSH after few days. No need of taking treatment for that.
Your vitamin B12 seems to be less as per provided range in your laboratory report. The decrease in B12 could be from dietary deficiency, malabsorption of B12 or pernicious anemia like cause.
If you have any gastrointestinal disease history like tropical sprue, enteritis, fish tapeworm infestation etc than it can be causative.
For pernicious anemia intrinsic factor estimation can be done.
For B12 deficiency, you need to be treated with cyanocobalamine injection once a week for six week.
For your sudden weight gain you can rule out Hormonal imbalance as suggested in my previous answer.
Avoid sedentary life style and do regular active exercise.
