Suggest Treatment For Infection After Circumcision
I'm 28 years old and I did my circumcision last Friday in an Islamic Sunnath House. The entire procedure took only 10 mins. 2 days after that I went to the same place for dressing and I must say, it was very painful to remove the previous dressing, since it was stuck in my Skin firmly. The Doc said, my body is very hot and that is the reason why the dressing got stuck to my skin. 1 day later I removed the dressing that the doctor did and it was very painful as well. I just had 3-4 medicines for first 2 days and then after the dressing change, the doc gave me an oinment - Chitomesh - SF 10g, and a tablet - Moxmac -CV 625(only for night).
The doctor recommended me to take bath and clean the wound using dettol soap, every day morning and evening and apply the ointment. Its been a week and I could still feel some pain there whenever I move. There were no stitches or anything made during the surgery. I take bath once everyday and trying to clean my wound everyday(its been 2 days). Every day, a layer gets formed over the wound and it gets removed since I'm taking bath.
1. Is is ok to take bath once or twice a day as recommended? Will it help healing process?
2. My body remains hot always, because I dont drink enuf water since childhood. Now even if I drink I urinate often and my body still remains hot. Will it impact the healing?
3. I still have pain over the wound. I shaved the hair on and around the penis for this surgery, the hair is growing gradually and is punching on my wound. What to do to avoid this? Will this impact anything?
4. Is the oinment and tablet enough for healing?
5. I struggle to walk even without clothes and dressing over my wound(inside my house). How long will the wound be painful?
6. A layer gets formed over the wound, few hours after taking bath(Ofcourse I put oil and oinment). Like a paste - Is it normal and should I do anything?
7. My Muslim friends asked me to put Parachute Oil on my wound and I m doing the same now. Is it good for healing?
Thanks in advance for your help and Support, Doctor. I'm really scared since it is still a bit painful whenever I walk. Please help me understand this. I have attached one picture as well.

Thanks for your queries, and the pictures.
Read the history seen the 2 pictures you have provided and understood your problems.
I would answer your questions as per the numbers you have provided with necessary explanation wherever necessary.
First of all do not worry, this has happened for a few reasons:
You are an adult of 28 years old. Circumcision in the child and adult respond differently as in children the healing is very fast and within a few days it is complete.
But in adult circumcision the healing is slower, there can be the morning erections making the skin edges to get separated and hence take time to heal as well as there will be more pain.
1. It is OK to take bath once a day as it keeps the area clean and does not allow the bacteria to grow more than usual as there is an would there.
2. Body being hot will not impact healing more. Yet you should drink plenty of water as more urination will not allow the infection to travel up to the urethra.
3. Pain will be there as I explained the reason above. Yes, the growing hair will cause punching and pain on your wound. This will not impact anything as such but irritates a lot in an already painful area. The Liquid Povidone iodine which I will tell how to apply will reduce your problem by making them stuck to the skin most probably.
4. Yes, the Moxmac 625 is very good; you have to take it three times a day for 5 days. Take something like Ibuprofen thrice daily which will help in pain and inflammation.
In spite of the ointment I would advise you the following:
Apply Liquid / Lotion Povidone Iodine (Betadine or so) 4 times a day to the whole area, let us dry up. This will help the cleaning and killing all the bacteria in the wound and the skin around. Easy to apply. Does not cause pain. You can apply a single layer of gauze if you have (you can get sterile gauze in a pack from the Medical store- if available), apply a single layer over the wound and soak it in Betadine Liquid. Apply Liquid Betadine before you change the dressing as a soaked gauze comes out without the pain and bleeding and it also takes the dead tissues with it .
5. Once you apply the gauze as told, the walking around will be easy.
Wound may be painful for few more days.
6. It is normal for the layer to form as this is the serum and the tissue and all. Stop oil and ointment and see the results.
7. No Parachute oil please at the moment , It is to be applied once the wound is completely healed.

There are few more points I would like to add that you have to discuss with your Doctor to get a ''prescription'' from him as this is a prescription medicines:
Amitryptilin at bed time may avoid morning- erection for healing to be hastened.
Take Multivitamins , they do help in healing.
The second picture shows a gap, Let us see how does it respond to treatment. A single stitch can help for early healing - to be done if the would gets cleaned up and the gap still persist. This may not be necessary.

Day by day, the pain and burning sensation is increasing and today, a layer of my skin has started coming off - both in the sensitive tip also in other areas.
Also, I'm afraid that there might be mucous formations in the wound. I'm attaching latest pics for review. You clearly see skin coming off here n there and white patches in the wound, which I feel like mucous.
Please assist.
The Necrotic tissues need excision.

Hi.
If the pain and burning sensation is increasing, you may need a surgical debridement of the necrotic tissues properly by a Surgeon as the layer of the dead necrotic tissue is coming off.
There can no be mucus formation, this can be a necrotic tissue.
Seen the pictures you have provided.
This is the necrotic tissue and needs excision for the healing of the wound properly. Please get this as soon as possible.
The skin that is coming of the tip - the glans is not to be worried about as this is the dead skin that comes off once the glans is exposed to the air, the upper layer peals off. You can apply the ointment you have for soothing purpose. Please do not touch it, do not pull it out.
