Question: I want Dr. XXXXXXX XXXXXXX , M.B.B.S., MS OB/GYN, FMAS+DMAS to answer my question:
In continuation of our discussion I have some question to Dr. XXXXXXX
1. What is best position to take Gynogen HP 75 IU injection, arm or buttock?
2. Is taking clofert and duphaston cause of painful Menstruation?
3. If pain is too much can I take ivoral forte ?
4. Please tell me one medicine for constipation.
5. Should my husband to do Semen Analysis after three months of previous test?

Thanks for the query ,
I would like to answer one by one.
1. The best position for the Gynogen HP 75IU injection is the buttock ideally over the upper outer quadrant of the buttock.
2. No, cleft and duphastarn intake has no relation to pain during menstruation at all , infact the pain can possibly be due to either fibroids, adenomyosis or even endometriosis, which should be ruled out if severe pain is experienced.
3. If the pain during menses is in excess the best med would be TAB MEFTAL-SPAS Thrice a day for next 3 days. Ivoral forte , if you have spelled correctly is an anti-parasitic medication and have nothing to do with dysmenorrhoea.
4. For constipation you may use TAB Bisacodyl 10mg once at night for 2 days or you may use SYP CREMAFFIN 2 tablespoons each night for 3 days. this should help along with adequate water intake during the day.
5. A semen analysis test is considered valid for at least next 6 months , so he need not get it repeated if the previous reports were normal.
All the meds would have to be prescribed in writing by your physician.

Dear Doctor,
Thanks for your reply. As you know during Nov 2015 my TSH was 8.3, after taking Thyronorm-50 for 6 weeks my TSH was 3.49 and I am taking Thyronorm 50 continuously. From Nov 2015 upto XXXXXXX 2016, 3 months I have been taking Thyronorm-50. I was not anymore Thyroid patient. I tested TSH as a pre-pregnancy test. Now I am feeling some pain in my throat, what could be the reason? Is my TSH going down below limit or due to weather? You told me TSH should be tested in 3 months cycle. Shall I test TSH now and tell you result? Kindly reply me please.

Thyronorm To Be Continued for long term.

Detailed Answer:
Hello,
If you are maintaining a level of TSH 3.40 on thyronorm 50 microgram daily then this is the suitable dose to be continued throughout. You may not get treated completely so soon as probably thyronorm would have to be continued for longer periods. The present report of 3.49 was because you have bene taking thyronorm and not naturally , so don't think of stopping them at all.
The ideal range of tsh to be maintained during pregnancy is between 3.2 to 3.8 and which is normal hence in your case, so need not be worried and continue the medication as you are continuing. This is a long term treatment and you have to understand this, it would be continued even till the term after you get pregnant.
