Brief Answer: combined insect bite and infection Detailed Answer: Hi. Thanks for nicely snapped photographs and your description. This is a classical cellulitis and the probable reason is combination of the insect bite and scratching enough to cause a break in the skin lining. This has resulted in the combined picture of cellulitis, hence the pain and tenderness, redness and edema. I would advise to take a combination of an antibiotic with antihistamine and steroids. It would be better to consult a Doctor to get prescription based medicines as per his age and weight. You can apply locally liquid povidone iodine to make the area sterile so that the chances of further secondary infection are reduced.
