Question: HELLO DOC-HAVE HAD ANOTHER EPISODE OF THIS PHENONEMA A SHORT TIME AGO. THE 2ND TIME IN A WEEK. AN INTENSE RUSH RUNNING FROM BELOW THE EARS INTO BOTH SHOULDERS & DOWN INTO BOTH ARMS. MILD PAIN AND GREAT WEAKNESS. SENSE OF DIZZINESS. HAVE HAD VERY HELPFUL LAMINECTOMYS IN CERVICAL AREA AND LUMBAR SACRAL APPROX 14 YEARS AGO. KEPT ME IN GOOD STEAD. I HAVE ARTHRITIS HEAD TO TOE. LIVE WITH LOTS OF DAILY PAIN DUE TO SUCH. HAD TB AS A YOUNG NURSE IN 1970, AND AS A RESULT THE RHEUMATOLOGIST SAID THERE IS NOTHING THEY CAN DO FOR ME. CANNOT RISK THE NEW BIOLOGICS. AM ANAPHALACTICALLY ALLERGIC TO MOST MAJOR ANALGESICS AND NSAIDS. AFTER A 3 WEEK BOUT OF UNRELENTING PAIN, THE ER DOCS TRIED MS 4MG AND VALIUM 2 MG. IT BROKE THAT SYNDROME THANK THE LORD. WAS ON BOTH ABOUT 4 DAYS AND PROBLEM RESOLVED. I HAVE MORPHINE AVAIL FOR VERY BAD PAIN BOUTS, BUT USE IT SPARINGLY. PERHAPS 1/2 TAB 10 TIMES IN THE LAST YEAR. I AM CONCERNED THIS MAY BE A NEURO HARBINGER. ALMOST WENT TO ER BUT AFTER LYING DOWN FOR 5 MINS OR SO IT PASSED JUST AS IT DID LAST WEEK. SORRY TO GIVE YOU SO MUCH INFO, JUST CANNOT TELL WHAT IS IMPORTANT HERE. I KNOW-TOO MUCH INFO. NOT LOOKING FOR A DIAGNOSIS HERE BUT
PLEASE ADVISE AS BEST YOU CAN. ANY IDEAS? WILL SET UP APPOINTMENT ON MONDAY, OR SHOULD I ER IF IT HAPPENS AGAIN? HAD TIA A FEW YRS AGO-RESPONDED WELL TO MEDS. ALSO HYPERTENSION AND TYPE 11 DIABETES ARE WELL CONTROLLED. MANY THANKS XXXX

Dear Madam,

Thank you for posting your query on HCM.

Well looking into your issue since you mention that the intense sensations affect both sides during the episode so it is less likely to be TIA although it cant be ruled out . But it looks more like syncopal attack or autonomic seizure to me .

Whether you visit ER will depend on the episode itself like if its duration increases or other symptoms like urinary incontinence occurs then yes going to ER will be must . At present I will suggest you to see a neurologist and discuss the same with him .

Regarding arthritis you havent mentioned which type of arthritis you have , only after that I can help you out with its treatment .

THANKS DR SHRUTI-I WILL FOLLOW AS YOU SUGGEST. HAVE AN APPOINTMENT THURSDAY AND WILL ASK FOR A CONSULT. AS TO THE ARTHRITIS, I BELIEVE IT TO BE OSTEO. TEST FOR RHEUMO ARTHRITIS WAS DONE TWICE. ONE CAME OUT NEGATIVE AND ONE WAS POSITIVE. AM MOST APPRECIATIVE. FORGOT TO MENTION THAT I DO HAVE URINARY INCONTINENCE. HAVE HAD IT FOR MANY YEARS. NOTHING NEW RELATED TO THIS SYNDROME.XXXX

Dear Madam,

Thanks for reverting back.

For osteoarthritis I wont suggest any active intervention as these are age related degenerative changes and nothing much can be done about them but keep on taking calcium and vitamin D supplement , they are good for bone health.

Urinary incontinence can occur in ladies at your age but in your context if it occurs with episode of those intense sensations then only its important , otherwise it would be just age related issue .
