Question: I am 56 years old and haven't had a period since I was 52. recently I have had throbbing pain that comes and goes in my genital area. I have not had sex in about 2 years since my husbands cancer has left him impotent. What could cause this pain and also muscle spasm similar to those I used to have prior to an orgasm?

The throbbing pain that comes and goes in the genital area and the muscle spasm is likely to be due to a mild nerve root irritation due to spondylottic changes in the lumbar spine. This is an age related degenerative change in the spine.
The diagnosis can be confirmed with the help of an MRI scan. Right now you can take mild pain killers in combination with muscle relaxants after consulting with your physician. Avoid bending forwards and lifting any weight. Do consult a gynecologist to rule out any local pathology after an examination.
Taking methylcobalamin supplements will also help.
