Suggest Treatment For Irregular Periods And Bulky Cervix
Attached the reports of my sister's scan. She has irregular periods. She didn't get her periods this month on the expected date and she was prescribed 5 days of medicine. She still didn't get it. Her doc advised her to take a scan. It says "bulky cervix". Is it something to be worried about?
irregular menses.

Hello,
Thanks for the query to HCM,
FIRSTLY THIS IS A VERY VAGUE REPORT WHICH THE RADIOLOGIST HAS RENDERED. CERVICAL bulkiness is labelled as hypertrophied cervix medically and can be due to chronic cervicitis or infection but that is not the cause of her irregularity.
the most common reason for irregularity of menses in young women is PCOS or polycystic ovarian syndrome with hyperandrogenic features, hypothyroidism or XXXXXXX hyperprolactiemia.
The uterus appears normal and so do ovaries, and hence it is important to get a hormonal profile with FSH, LH, SERUM TESTOSTERONE, ESTRIOL, DHEAS, 17OHP , TSH, SERUM PROLACTIN levels which would be able to clinch the diagnosis further.
AFTER TAKING REGESTERONE pills , the menses should occur in next 5-7 days but if not then can wait till 10 days as well. But if still not then a D&C can be done if she is married already or sexually active, else a repeat course of regesterone can be offered.
But is the bulky cervix a cause for concern? She is not married yet and she is not sexually active. We googled bulky cervix(a mistake we understand) and all the results point out at cervical cancer. We are a bit worried.

She would need a course of empirical antibiotics for 10 days which can help her subside infection and a repeat ultrasound after 10 days to see teh cervical condition. Irregularity is not due to hypertrophied cervix and cervical cancer is not seen at her age when she is sexually not active.
