Suggest Treatment For Irritable Bowel Syndrome

Question: i am suffering from ibs triggered by obssessive personality disorder. My pshyciatrist has prescribed me three different SSRI's daily. I am feeling quite well now. But i fear that after completing the course, the effect of the medicine would fade away after several months. So is it a permanent cure or am i right? And if it is true what should i do to completely remove the problem?

Hello,
Its good that you are having satisfactory response with treatment. There are certain points which should be remembered and followed to prevent relapse and remain symptom free-

1. complete the course as advised by the psyhiatrist
2. you should start lifestyle changes to prevent relapse. IBS means there is some kind of poor stress coping and anxiety prone behaviour which disturbs gastric system - these can be improved by practicing deep relaxation techniques.
3. staying away from junk food, use more healthy diet and maintain sleep hygine
4. the most important aspect of treatment is therapy- this will help your OC behavior. Though OC is still a trait in your case as you mentioned, but still therapy will help you remain symptom free even after stopping drugs.

What do u mean by therapy? And can u suggest me some relaxation techniques which could help? Also the ssri which i have mentioned have different salts so does they have different reactions or just they had been given for the mightyness of their different suitability to the body?

Hello,
Therapy is a non medicative approach to treatment which is decided depending upon the need in particular case. In anxiety prominent patients therapies like - deep breathing, JPMR, etc. help a lot. They make the patient have control over their anxiety prone nature and obsessiveness.

You can learn them from your psychiatrist. Even going to a gym and properly following a good exercising routine can help a lot.

Secondly, if you want to know my opinion about your drug regimn- I dont like it.
It is a combination of various SSRi. Your doctor could have raised the dose of one med adequately and tapered and stopped others. There is hardly any point in taking three meds from same group (the mechanism is same in all of them anyway).
But coz you are keepin fine on them right now so, I didnt mention anything on this matter. Dont worry as there is no harm.

Its beter that you continue on them and then start tapering with one of them ( which will be considered least effective amongst the three by your treating doctor).

Are there any side-effects of the prescribed medicines? I asked this because today, I felt some kind of shakiness in my body, during sleep, which got me awake. And for some days I have been feeling hotness in my heels. So please tell if it is related to the existing problem or i should consult some other doctor.

Hello,
Sudden shakiness in body espically during sleep is a common finding in normal individuals also. In your case it can be either normal or just due to anxiety. If these jerks increase then you can further increase the dose of paroxtine to 37.5mg (and may reduce taloprex to 15mg). This is not a serious issue so dont worry so much. It will settle with dose management.

Secondly, incomplete evacuation. It is one of the commonest complaints of people on antidepressants. Ideally, the doctor should ask the patient to increase the consumption of salad (3-4 servings in a day), and increase the amount of liquid (sips of water every 10-15min; to any liquid like juices, fruits, etc.) in the begining of the treatment. By doing it from the starting this bowel complaint dosent build up. You can introduce these chanes now. For some days you can take - cremaffin syrup 1-2 tsp at night.

Thirdly, for buring feet- you can get blood sugar levels, blood pressure readings and Vit. B12 levels. If found disturbed, correcting them will help.
Take a good neuronal supplement for one or two months. Your doctor can advice you on which are available locally.
