Detailed Answer:
Hello!
Thank you for asking on HCM!
Regarding your concern, I would like to explain that there is not a specific antihypertensive therapy against systolic blood pressure. This is a condition, which usually needs medical therapy, besides lifestyle modifications.
Based on different studies, thiazide diuretics (hydroclorthiazide) and calcium channel blockers (like amlodipine) are the drugs of first choice for this condition. The desirable target level of systolic blood pressure, according to your age would be around 140.
Other recommended drugs effective in high systolic BP would be ACE inhibitors (enalapril, lisinopril etc.)and ARBs (valsartan, losartan, etc.).
So, as long as your systolic blood pressure is around 140mmHg, I would recommend continuing with your actual therapy.
If your systolic BP is higher than 140mmHg, I would advise consulting with your attending physician to consider the possibility of adding another drug like hydroclorthiazide or an ACE inhibitor/ARB.
It is also very important to make some modifications to your lifestyle:
-modify your diet (reduce salt intake, reduce caffeine intake)
-perform a lot of physical exercise
-loose weight ( if you are overweight ).
Regarding your chronic back pain from spondylosis, I would advise some tips to help you with your pain:
1- Physiotherapy is very helpful in relaxing the muscular spasms, caused by chronic spondylosis and inflammation.
2- Loose weight: It is very important if you are overweight to loose some pounds, and relive your lumbar vertebral column from this extra weight.
3-Avoid sitting position for long periods.
4- Avoid straining physical activity and holding heavy objects.
5-A lumbar support can be helpful in relieving the pain and helping you correct the wrong staying position.
If all the above tips, are not effective, you can try medical therapy for chronic back pain (antidepressants like SSRI class or antiepileptics like gabapentine or pregabaline).
In case of progressive worsening of back pain or difficulty walking, a neurological consult and a lumbar spine MRI may be necessary to exclude possible canal stenosis.
Hope to have been of help!
Feel free to ask any other questions, whenever you need.
Best wishes,
Dr. Iliri
