I read your problem and saw the photograph.
You say you have Gyno but the photo did not suggest the same. Do you do any strenuous exercises daily? The photo also did not suggest that there is any infection. Is there any discharge or recent swelling? Do not worry and see a Doctor who can examine you and find out whether there is any infection or a cyst under the nipple?
Secondly, since when do you have Gynecomastia and have you seen any Surgeon for the same?
I suggest you see your GP or a Surgeon who can examine you and find out what is causing the problem. You might require a Dermatologists' opinion about the itching and might require some soothing agents like Calamine lotion for the itching.

Hi,
It is very very unlikely that you have breast cancer.
Cancer usually occurs in older age and presents with a lump which is usually fixed to underlying tissues in males. But, it would be better to get it ruled out and that is why I suggested you get yourself examined by a Surgeon who would be able to rule out cancer. If the doubt persists a minor investigation like an FNAC(Fine Needle Aspiration Cytology) can rule it out. In this, a small needle is inserted into the lump and some cells are taken out which are then examined under a microscope to see what the growth contains.
I still think you need not worry and see your GP or a Surgeon who can do the above and get to the root of the problem.

Hi,
In cases of Gynecomastia there is almost always a swelling or lump behind the nipple made up of connective tissue or subcutaneous fatty and areolar tissue. Therefore, it is normal to have a lump behind the nipple in a case of Gynecomastia. But, it would be better to get it tested by means of an FNAC as I mentioned earlier or if that is unable to comment then a Biopsy might be required.
You should go ahead with that specially if you feel like it is turning cancerous.
So, see your Surgeon and he can get the test done after he examines you.
The results of an FNAC come within 24 to 48 hours, so you will not have to wait long.
