Brief Answer:
Eczema
Detailed Answer:
Hello,
I understand your concern. I think that, given the medication you have used hasn’t provided any release, a fungal infection can be safely excluded.
In order for me to narrow down the possible causes I need to know whether there is a rash associated to this and if yes, what does it look like? Does it spread beyond the scrotum? What time of the day is the itching more intense?
Please provide this information so I can give you a more accurate answer.

I considered eczema as a possibility. So I discontinued all antifungals. Thought maybe I was treating an infection that wasn't there. So I tried using just Gold Bond Medicated lotion. But the itching intensified over the next couple of days. Making me think once again maybe it is tinea, and I just can't shake it because of the ocrevus l take . It is a B cell depleting medication. Prior to that I was on Tysabri, a T cell depleting medication. So as you can see my immune system is compromisedTo answer your question, there is no rash. But the itching has spread to the back side of my upper left leg. The itching has no specific time of day
Brief Answer:
Could be a neurological problem
Detailed Answer:
Thank you for the additional information. I think eczema and an infection may safely be excluded, given the medication used.
The only explanation can be neurological, which can also be supported by your MS diagnosis.
Itching can sometimes be a sign of mild nerve damage or nerve regeneration. I recommend you talk about this to your neurologist. If you haven't already had a CT or MRI, I recommend you do a lumbar imagery for further investigation.
I hope this helps.
Wishing you good health and all the best.
