Brief Answer:
Seems to be lichen simplex chronicus
Detailed Answer:
It seems to be lichen simplex chronicus which is a chronic itchy disease of the skin characterised by thick hyperpigmented plaques on the skin.
In such cases I recommend my patients to apply high potency steroid like Clobetasol propionate cream or betamethasone propionate cream on the affected area twice daily along with antihistaminic like tablet hydroxyzine 25 mg twice daily.
Steroidal preparations are prescription drugs and you need to see the doctor in person to get them.
If the itching still continues I recommend them to go for intralesional steroid injections every 3 weeks.
The treatment is continued for a long time probably 3-6 months depending on the response.
