These white layer and small dots s might be due to some irritation due to soap or due to some allergy or pearly penile papules but I will need to see those dots to arrive at a certain conclusion.
I will need to look at the head penis to check whether there is fungal infection or something else as you have already tried anti-fungal cream with fluconazole tablets. There is an option to upload pictures on this page. You can utilize it.
And also before proceeding further,I would like to know,
Is there any pain or burning or irritation over the glans?
Do you often use condom?

Hello,
Welcome again and thanks for sharing pictures.
Based on the facts and pictures that you have posted here,it is likely to be due to infection of glans penis (balanitis or balanoposthitis) and I really doubt about fungal infection. This infection is likely because of some allergy or inflammation (may be due to soap cleaning)
Gently retract the foreskin and soak in warm water to clean penis and foreskin.
Avoid using soaps further as they may irritate the skin and make the inflammation worse.
Take broad spectrum antibiotics small course like cefixime 200 mg twice daily for a short course for 3 days.
Topical antibiotic with steroid ointment like Betamethasone and Neosporin (Betnovate-N) for local application over glans twice daily.
But for oral antibiotic,you will need to consult some local doctor or dermatologist who will better know about it.
Discontinue using topical and oral anti-fungal drugs,as it is very unlikely to be fungal infection but you can continue with vitamin and minerals.And yes,there is no need of any diet change here as diet has not much role here.
