Suggest Treatment For Itchy Swollen Scrotum

Question: Very itchy, swollen scrotum. Top layer of skin flakes off. Tried Viaderm, Lotriderm and mild Topicort. Was told by one doctor to shave, tried that, no dice. Another told me not to shave because of the metal in the razor. Tried that, same thing. Dermatologist recommended switching to boxers/boxer-briefs (non-red/black) and dove soap. Tried that, ditto. This has been ongoing for years, keep seeing doctors, keep getting useless advice and worthless prescriptions. Flesh appears red and swollen. Any assistance would be appreciated.

As per details, there are following possibilities for your symptoms :
1 ) Tinea : it is fungal infection of skin. It is characterised by red itchy scaly rash over involved area.
2) Scrotal dermatitis : it is characterised by ill defined red colored itchy rash usually extending to involve anal area also.

Good day and thank you for your prompt response.

Actually there is an area in the middle of the upper buttock area that gets itchy and if scratched, swells and weeps (stays swollen and weeping for at least a day) but the joining tissue between the two areas doesn't seem affected. The scrotal area, when itched, will intermittently weep as well. The 'fluid' in these cases, when rubbed away, quickly becomes sticky and has a foul odor.

The creams (viaderm, lotriderm and topicort) seem to calm the affected areas long enough for them to heal (if applied for a couple of days) but as soon as treatment stops, symptoms come back almost immediately. I will post pictures soon.

As per information you have provided, it is more likely Scrotal dermatitis.
The ointments that you have applied are actually triple combination of topical antibiotic, steroid and antifungal.
These work for a while then they stop showing effects.

I'm sorry to have to show you this, it's a picture of the upper middle buttock region, this is an early stage of inflamation, I'm unable to upload a picture of the scrotal area as I had recently treated it. I can, if time permits, send it at a later time.

Again, I apologize for the picture. I loath showing ANYONE things like this, let alone repeatedly, which is why I'm frustrated that no one else has taken the time to just fix the issue.

I have seen the attached photograph also.
No worries, we as doctors are here to help you only.

As per details, the most likely possibility for your symptoms is a condition of skin called Tinea cruris et corporis. It is fungal infection of skin involving body and folds.

Various precipitating factors are Diabetes Mellitus, excess sweating, hot humid weather.

I will advise you to get your blood sugar levels done.

In such cases, I prefer to advise topical antifungals like clotrimazole/miconazole cream. Also oral antifungals like fluconazole/ terbinafine are also started.

Keep the area clean and dry.
Daily change and wash your clothes.
You can take antihistaminics like levocetrizine for symptomatic relief of itch.

You should strictly stop applying those combination creams.
These may give temporary relief but would worsen the condition only.

You can share my opinion with your doctor.
