Detailed Answer:
Hello,
Thanks for your reply
Looking at your description this could be due to Acid peptic disease which can be caused by medicines he has taken for fever like paracetamol and anticancer chemotherapy which increases acid production in the stomach leading to loss of appetite and excess gas.
Apart from this, during cancer chemotherapy, chemical mediators are released which may cause inflammation of the bowel as well anorexia (complete loss of appetite)
If you can upload CT Scan report which was repeated, I will be able to guide you even better.
If I were his treating doctor I would have advised him to take Pantoprazole or omeprazole group of drugs once daily for 10 to 15 days. Bowel inflammation needs monitoring and further evaluation.
Give him liquid diets with fruits in between.
I hope with this his appetite will improve
Please don't hesitate to write back to me if anything not clear

TPN is best and only option to feed him

Hello
Thanks for your reply
I have seen CT scan report, looking at report and detailed history, if he is not able to eat anything orally than Total parentral nutrition (TPN) is best and only source to feed him
Bowel inflammation takes time to heal, it require regular monitoring you should ask your doctor for right composition of the TPN solution.
Hope this answers your question, please feel free to ask for more information, I will be happy to help you

He may require TPN for 2 to 3 months
Detailed Answer:
Hello
Since CT report is suggestive of diffuse bowel wall oedema and thickening of the distal ideal loops along with previous hemicolectomy and nephrectomy, it's quite obvious that his condition is critical so he may require TPN for 2 to 3 months until bowel heals completely and appetite returns.
Please observe strict hygiene and cleanliness during TPN
Hope this answers your question Please feel free to ask for more clarification I will be happy to help you

Please consider these explanation
Detailed Answer:
Hello
Since he has metastatic colon cancer and he has given the course of anticancer chemotherapy
During chemotherapy various chemical mediators releases which can damage the entire body including intestine as well
As such he was having high grade fever so another possibility is intestinal infection leading inflammation of the bowel wall
As of now fever has subsided, so there are very good chances that it may heal up.
Nutrition is must for speedy recovery so please discuss with his doctor for TPN and it's right composition.
Hope this answers your question Please feel free to ask for any more assistance I will be happy to help you

He feel better with TPN?
U don't think then that this is cancer related? Cancer getting bad .?
Well after 3 rounds of chemo his cea has been dropping each month and some small mets in the liver have disappeared leaving only 3 liver mets now and 3 small peritoneal mets
He will definitely feel better with TPN
Detailed Answer:
Hello
Thanks for your reply
Definitely he will feel better with TPN because body also needs all the nutrients to keep it active and functional
without the nutrition his condition is going to deteriorate further so i am in favour of TPN
As I wrote in my previous answer that bowel inflammation can be caused by metastasis , chemotherapy induced chemical mediators or infection ( he was running fever)
So in my opinion a operated case of colon cancer needs close monitoring and better nursing care, overall prognosis depends upon patient care, patients ability to fight against infection and common ailments ( immunity)
I assure you here that with good nutritional supplements and better nursing care will definitely help him to recover fast.
Hope this answers your questions

The mets start growing again or spreading?
It may relapse which again depends on number of factors
Detailed Answer:
Hello
I really appreciate your concern
Relapses may occur on discontinuation of chemotherapy or even with therapy as well
Overall prognosis depends on number of factors like
immunity of the patient
nutritional status and proper nursing care
any triggering factor or acute illness like infection,
So in my opinion he needs close monitoring and regular follow up to keep a close watch or relapse
I hope you will find my replies and conversation helpful and informative,
Thanks a lot for your questions
