Question: My three year old was punched on his back left side near his ribs by his cousin. Hes complaining they hurt. What could be wrong. There is no outer bruising

The first thing to be done is to get an X Ray chest to rule out any bony injuries or rib fractures. Even if the X Ray does not show any fracture, the bones could still have got strained and hence cause the pain. The soft tissue (consisting of muscles, fat etc.) do not show up in an X Ray and could also be present. There need not be any outer bruising for the same.

I would suggest you to apply hot compress over the affected area. It will help in reducing the pain. Analgesics may be considered only in more severe cases.
