Suggest Treatment For Lesion On Liver

Question: Hello Doctor,
I am 32 year male, last year there was a lesion in my liver I had tested for AFP it was negative and after that I was fine and started taking liv 52. But in between I stopped.Now in this year i had a chest pain for few days and cheeked fof LFT . My bilirubin is 1.7 now rest all the enzymes are normal. But i m feeling tired during the day.Now started liv 52. I dont have any symptoms of jaundice and Naucia! some times light pain in chest i use to have.
Can you please solve my doubt.

I would like to know exactly what was the lesion in the liver.
How was it detected? What were your symptoms?
And besides AFP, what other investigations were done?

I will need to know the above details especially to know the reason why bilirubin is raised.
However since enzymes are ok, I dont think you really need to worry too much.

As for the chest pain, there are many reasons why the chest pain can occur-
1, in view of your history, I would say even severe gastritis can cause the chest pain.
2, there may possibly be a chest or pleural infection. This may result in the pain too
3, and then there is the obvious cardiac component that we need to know.
In view of this, can you think of any symptoms that you may also have and have not mentioned in the query? If so please mention it.

The lesion was an hype echoic lesion , and radiologist said that it was a hemangioma (0.6cm).
I had a second opinion in that, fif ultrasound scan again with another doctor and also AFP test for that which was negative.
I dint have any symptoms , it was a regular check-up for which I took ultrasound scan.
I had a LFT at that time and my Bilirubin count was normal.
(this was the history)

Following Symptoms I have

Recently (one month before) I had a small chest pain while breathing and I went to doctor .
I had a check-up for BP and Lipid profile BP was 130/80 and Lipid profile was normal.
I was feeling like something stucked in the throat , but I could swallow the food easily.---(I have this feeling some time now and then but most of the days)
I have a burping/belching 3 times after the food of even if I drink small amount of water.
Sometime feel tired after having food (for an hour after which I will be OK)
Some time I feel energetic and perfectly OK.
I use to get lot of mouth ulcers ( family history)
Depression (some times)
Sometime feeling sleepy during the day


Symptoms which I dint have till now

Nausea
Stomach pain
Dark urine
Headache
Abnormal stool
shortness of breath.

Family history

My family doesn’t have any history of any diseases.
My dad had kidney stones.

I had a check with my family doc in my native, he suggested me to take Liv-52.
I have started it, but I see slow improvements.
I am having barley water every day.

Hope this information is sufficient to make some conclusions.

I am quite satisfied with your response.
In my mind, there is almost no doubt that what you are suffering with is GERD.

GERD is gastroesophageal reflux disease.
In this basically you have acidity. The excess acid may sometimes come up to the food pipe. And this may cause fleeting chest pains that you have.
This also explains the belching and burping episodes and the mouth ulcers.

I will suggest a course of antacids called esomeprazole. these can be taken once daily 1/2 hour before your first meal.

For the sake of safety, I would suggest getting an ecg and echocardiography as well.

-->What could be the cause of increase in Bilirubin ?

First and foremost, yes esomeprazole and liv 52 can be taken together.

Second, GERD can make you feel tired because one is not comfortable overall. And eventually many people do not realize it, but have reduced some diet to be more comfortable.

Third, the increased bilirubins are a part of hemangioma detected in liver and not a part of raised bilirubins.Besides, Liv 52 is sometimes known to raise the bilirubins too. However, since your enzymes are ok, i dont think you need to worry.

Repeat the test in a week, without having taken any medicines in between and this may be normal. A repeat usg may also be significant.

--Please let me know esomeprazole shoud be taken 20mg or 40mg.

--Since few days i couldn't able to sleep properly in the night.

Hope this is a problem with GERD.

Hello Doctor,

I am taking the tablets and the GERD problem seems solved.
But i have noticed that i am having fatigue during the early morning and light headache.
But after the lunch i noticed that it will slowly go. and i can feel bit relaxed.
My appetite also increased and i feel if i don't have food when i am hungry, i am feeling like loosing energy.
What could it be.
I have not checked my Bilirubin again.
Can you please clarify on this ?

I think it is good that the GERD part has settled
Sometimes it takes time for the rest of the syndrome to settle.

However I still think that repeating a LFT along with CBC will help us know where we stand.
Also, checking FBS and TSH will be a good idea.

I will take those test during the weekend.

Yes , i have few other symptoms that i feel that heart rate is increasing in the morning even though i use to go to brush/toilet.
Heart beat will be normal once i take rest.
Feeling like lost control in the neck sometime.

I don't have any sense of nausea/Vomiting.
I don't have symptoms of feeling Feverish.
I hope it will take long time to recover , since it's a problem with Liver.
