Suggest Treatment For Lesions At The Back Of The Neck
Question: Hello Doctor,
My father is getting the attached lesions on the back side of neck. He is suffering from diabetes already and undergoing medication for the same since last 6 to 7 years. Could you please prescribe appropriate medication. I have enclosed a copy of photograph of the same for your review.

Acanthosis nigricams with a patch of lichemoid dermatitis

Hi. Thank you. I have reviewed the Image.
He has acanthosis nigricans along with a patch (2cmX2cm) of lichenoid eruption, on the back of his neck. Acanthosis nigricans is more common in diabetics and overweight individuals and presents with dark, velvetty texture of skin of neck folds. Acanthosis is quite stubborn to treatment, however, topical tretinoin 0.05% cream, once daily at night, has been tried with positive results in acanthosis nigricans. Specifically for the lichenoid patch, I suggest him to use a topical potent steroid cream e.g clobetasol propionate 0.05% cream (tenovate cream), twice daily, for 2-4 weeks. Since this lichenoid patch may also be itchy, therefore he may also take a tablet of cetrizine 10 mg once daily, in evening for symptomatic relief from itching.
