Suggest Treatment For Lesions In Pubic Area Diagnosed With Genital Molloscum

Question: Hi,

I was diagnosed with genital molloscum in late April. I had two cryo treatments in early May, and my last one in early XXXXXXX I thought I was going to be in the clear as of about a week ago, my pubic region looked like it had almost fully healed and the lesions almost gone, but I have now had a new flare up in a new area: in the creases of my legs, and along my scrotum. And this flare up looks really angry, and itches and hurts more than the one before.

I should also note that I've been using Aldara for the past month basically every evening. I used it across my entire pubic bone region, which is now almost clear, but not on my testicles or pits of my leg, which are now heavily infected.

My concern is that three of the patches are red, slightly pussy, and somewhat painful. After my last cryo treatment, that's how they looked, too, but these don't seem to be healing. I have to wait another 2 weeks to get in to see my doctor - what is your advice for treating the infected regions in the interim? For the record, I have been usin Aldara aggressively because this is really starting to affect me emotionally. I want this stuff to be gone and it looks like it's just spreading to new areas, and getting worse as it does.

Hello,
Thank you for posting on HCM.
I can understand your concern regarding the skin rash/lesions over your pubic region but its difficult to point specific diagnosis in absence of clinical examination.
From your history it could be contact dermatitis from aggressive use of Imiquimod cream (Aldara) or could be allergic reaction to healing of mollusca (Meyerson phenomenon) or could be simple ringworm infection.
Therefore for proper management, I insist you to post good clinical pictures to me so that I can help you in a better way.

Application of corticosteroid and antibiotic combination cream

Hello,
Thank you for the pictures.
I have reviewed them carefully and I can see no signs of active Mollusca or any allergic reaction to Mollusca nor any clear signs of fungal infection.
Thus I have come to conclusion that the rash seems to be because of contact dermatitis of Aldara cream.
Nothing to worry as it will be resolved pretty soon.
As for now I would advise you to stop applying Aldara cream till complete resolution.
In such cases, I advise application of Hydrocortisone and Fusidic acid combination cream thrice a day over affected areas. Also sprinkle generous amount of clotrimazole dusting powder after applying the above mentioned cream.
I will also advise 5 day course of oral antibiotic like Cefadroxil to prevent secondary infection.
Avoid use of any soap over the affected areas for a week or so.
Take antihistaminics like Bendaryl as required to relieve itching.
I am sure you will be fine soon enough with the above treatment.
