Suggest Treatment For Loose Motions

Question: Hello Doctor,

My baby is 3 months old now with birth weight 3 kg. She has loose motions problem since 2 months. She is completely on mothermilk. But failing to thrive and Reducing substances found positive in stool test. We understand that it is Lactose intolerance.
Our paediatrician suggested Zerolac powder for a week but she doesn't drink as she is addicted to mothermilk. The loose motions still continue. She is just 4 kg now. We are worried a lot. Please suggest us what can be done for my baby to thrive or if I have to change my diet in order to stop her motions.

Hi...I have gone through your case in detail. I need some more information before giving you suggestions...

Practical issues till now -

1. Stool reducing substance positivity is normal in exclusively breast fed babies.
2. Your baby's weight is less than 5th centile according to growth charts.


Questions -

1. Did the loose stool start after any specific substance or cows milk was given?
2. Is there any blood in stools?
3. On average how many times the baby passes loose stools per day?
4. How us the general activity of the baby?
5. On average how many times she passes urine?
6. Does she have any eczema like rash over the body especially in flexural regions?
7. Is this your 1st baby... If not how is your other kid? Did he or she have similar problems?
8. What's the length of the baby?
9. Mention exact date of birth.

Answer all questions in order and I will suggest you the needful.

1.     Initially at the time of birth, few spoons of cow milk was given since I could not produce sufficient milk (Birth weight dropped to 2.5 kg within first 10 days). Then I started giving my milk but was insufficient and my doc suggested me to give NanPro1 powder. I would alternatively feed both my milk and NanPro1. This was only for first month (again weight raised to 3.4 kg). From 2nd month onwards, I started producing sufficient milk and stopped bottle feeding completely.
And the problem started from 2nd month. When contacted doc regarding motions, he suggested Zerolac powder for 1 week but my baby could not drink at all because of the taste as she was addicted to my milk.

2. No blood in stools. Sometimes watery or curd like white lumps.

3. Around 8 – 10 times a day.

4. Baby is active. I mean she sleeps well, smiles and responds to us, makes her own aaa aa…ooo oo sounds, plays continuously for 2 hrs.

5. Urination is also fine. She passes urine once every 30 mins or so.

6. She has no such rashes or allergies.

7. Yes, this is my first baby.

8. Current length: 55 cm, current weight: 4 kg.

Hi...I went through the history points you have provided -

I was expecting that there should have been a history of cow's milk and formula feed (this too contains cow's milk protein) given to the baby. I feel that your baby is having cow's milk protein allergy. Unusually babies grow out of this sort of allergy by 1 year of age.

My suggestions for you -

1. Mother should go off cow's milk protein completely. This means that you should avoid consumption of anything and everything related to cow's milk - like - milk/ curds/ ghee/ butter milk/ chocolates/ biscuits/ ice creams etc. Even while buying commercial food products, you need to see the ingredients and if they contain milk - do not consume them.

2. Feed you baby only exclusive breast feeds till 6 months of age and then start rice based feeds.

3. If this is followed scrupulously - the baby will stop having diarrhoea in another 3-4 days and then start gaining weight too.

4. If you feel your feeds alone are inadequate for the baby, then Zerolac is the only option.

Regarding my diet, yes, I drink milk twice a day but that is of buffalo's. Anyhow I am ready to stop drinking it.
And I am only having rice, bread and biscuits. But no cow products. However, I will stop having bread and biscuits too. Let me check if her motions stop.

Do I continue giving Zincovit, Vitanova D3 and Simyl oil.
