Suggest Treatment For Low BP And Uncontrollable Sugar Levels
Question: 61 yr old female, with out of control diabetes, BP 98/47 with lightheadedness, on cipro for kidney infection (spent Wed-Fri in hosp for low BP with syncope)

Blood pressure of 98/47 is low, but not that dangerously low. If less than, 90/40, i would consider it as dangerous. Because at this BP, body will struggle to maintain the blood suppply to vital organs including kidneys. If your BP is stayig low, ongoing infection ( sepsis) is a concern. Do you have the Urine/blood culture reports? Was Cipro started after the culture reports? Any nausea or vomiting?
Do you feel light headed on getting up from bed?

Went into ER with BP of 80/40, lightheaded and seeing spots (and with pain in abdomen and right flank--had appendectomy and complete hysterectomy ages ago). Input testing showed creatinine of 2.5 and low magnesium and no fever. Immediate IV of saline and magnesium reduced creatinine to 1.5, but later testing revealed UTI and kidney infection, so then IV cipro was started. IV fluids continued (enough fluid for me to gain 12 pounds). Discharged late Friday (BP 112/59), with cipro Rx to continue at home. Midday Saturday got lightheaded again (but no spots) and BPs of 85 to 98 over 42 to 57. Had some caffeine and got it up into 100+/60+, lightheadedness stopped then. Have continuing nausea (but no vomiting--just retching). Food tastes off (even chocolate!), so not eating much. Last bowel movement was Monday (I have IBS-C). Taking Linzess since Wed. and just started Colace. Scheduled to see cardiologist Thursday, GP Friday and Gastroenterologist the following Monday (8 days from now). Had ultrasound to check gall bladder and kidneys--Hospitalist said they were okay, but that I might want to have gall bladder out "just in case".

Thank you for the info. So it looks like a case of Uro sepsis. I was wondering if they did a urine and blood cultures before staring IV cipro ?. The fact that you are still symptomatic, it possible that the infection is still persisting. It is worthwhile to check the CBC and elctrolytes as well.
