Suggest Treatment For Low Blood Sugar Levels In A Diabetic

I am type 1 diabetic and 12 week pregnent.
My sugar was almost in control then in 10 and 11 weeks on lower side.now in 12 weeks I am experiencing high sugar. My fasting are normal but 2 hrs after eating it is 176 and 161 irrespective of doses and item I eat.
But in 3 hrs it drops down and subsequently falls to low sugar if I don't eat anything.
I am unable to understand this fluctuation.
Pls elaborate reason behind it.
Pregnancy hormones changes insulin sensitivity.
Detailed Answer:
Hi,
Thanks for your question.
Noted your concern.
In pregnancy in initial half there may be increase in insulin sensitivity especially in Fasting state leading to increased chances of Fasting hypoglycaemia.
As pregnancy advances there may be increased insulin resistance leading to postprandial hyperglycaemia.
There may be need to reduce dose of long acting insulin & increase the dose of short acting insulin.
This may be one of the reason for 2hours post meal hyperglycaemia & there after hypoglycaemia (near Fasting state).
In pregnancy there is a increased need for nutrition for development of baby.
That's why frequent small meals are usually suggested in pregnancy.
3 major meals (breakfast, lunch & dinner) & 3 snacks (mid-morning between breakfast & lunch, evening & at bed time) usually suggested.
This may help in reducing the chance of hypoglycaemia.
In such a case, I may suggest
- Consult your dietician and modify diet plan according to need in pregnancy & weight gain during pregnancy.
- Regular follow up with your treating diabetologist.
Hope this helps you.

Dr Abhay Mali.
