Majority of your reports are normal except Ultrasound scan done on 22nd March 2013 2 which suggest possibility of Appendicitis or Pelvic inflammatory disease.
Going by description of your symptoms of lower abdominal pain and loss of appetite during episode of pain possibility of Chronic Appendicitis needs to be considered. Polycystic ovary as report in one of the scan will not have such presentation.
CT scan or other scans have failed to conclusively attribute cause to your symptoms. In such scenario I would recommend you to undergo Diagnostic Laparoscopy (keyhole surgery) as that will help in definite diagnosis and management. Do discuss it with your treating Surgeon.

As suggested previously it could be either Appendicitis or Pelvic inflammatory disease. As the diagnosis is not established by noninvasive Radiological techniques further diagnosis is possible by Diagnostic Laparoscopy.
I would suggest you to discuss it with your Gynecologist.
Diagnostic Laparoscopy is almost like a day care procedure .

Consider adding an antispasmodic

Apart from antibiotics I would also suggest you the following
1. Add an antispasmodic like Drotavarine
2. Acid suppressant like Pantoprozole will help
You will need to discuss the addition of these medications with your prescribing doctor and get a prescription for the same.
Also do provide me the following additional information
1. Is your appetite normal ?
2. Are you having fever?
3. Are you presently having urinary symptoms
I would suggest you to discuss with your Gynecologist the need of diagnostic Laparoscopy which can be done as elective procedure.
