Could you please tell me the nature of the pain- pricking type, dull, colicky?
Also is it accompanied with vomiting or fever? Is it intermittent or continous?
Since your ultrasound is normal , it is unlikely that there is any problem due to the removed appendix.
Since we need to rule out infection, i suggest consult a physician and get:
1. Complete blood count/Hemogram
2. Urine analysis and microscopy (To rule out urinary tract infections)
3. KUB X-ray - showing kidneys,ureters, bladder will be helpful
A spiral ct scan abdomen pelvis will help finding out any problems in the internal organs which may be missed an ultrasound.
These investigations will help in diagnosing your problems.
In the mean while you can take an over the counter drug:
1.Tablet Omeprazole 20mg half an hour before breakfast daily one tablet for 3 weeks. It decreases the acid production in the stomach and can reduce pain occuring due to gastritis.

Since you are taking proton pump inhibitors since past 5 days , you need atleast 3 weeks for the medications to give you complete relief.
Since your ultrasound reports are normal, back pain due to renal causes is rare in that case. However, urine analysis and microscopy is required to rule out urinary tract infection and a KUB X ray.
Next, we should rule out orthopaedic spinal disc causes that can lead to back pain which can radiate to the thighs.
You can consult an orthopaedician for a spinal Xray lumbar region for the same.
Kindly apply a topical analgesic cream like diclofenac gel three times a day on the back and you can consult a physician for oral analgesics like Tablet Aceclofenac 100mg daily once after a good meal for 3 days. Do continue with the proton pump inhibitors that have been given by your doctor.

yes presence of gas in the abdomen can lead to pain. For fatty liver you need to start diet control by avoiding junk foods and fatty foods.
Opt for fibre rich diet , fruits and vegetables. Lower down on meat if you are a non vegetarian.
You can try this over the counter digestive syrup: Syrup Aristozyme(available in india) one teaspoon after meals three times a day. Exercise daily for one hour by brisk walking.
Drink a warm glass of water early morning after getting up from bed.
You dont require CT abdomen since ultrasound and endoscopy is normal.
You can visit an orthopaedician for lateral view of spine to rule out any disc problems that can cause back pain.
Drink lots of water for proper functioning of the kidney.
Rabiprazol(proton pump inhibitor- for gastric problems), aceclofenac(for back pain), rifaximin(antibiotics) tablets are absolutely fine .Continue that as advised by the doctor.

Firstly, its been four years since you have been operated. Stump appendicitis is a very very rare condition which occurs due to accumulation of fecolith (faeces) in the stump left over after appendix operation. Plus you donot have typical symptoms like fever, vomiting and acute pain abdomen.Also your ultrasound is normal so there is no reason to worry.
You are experiencing these type of symptoms cause your diet must be consisting of fatty sticky junk food. This in future increases chances of digestive diseases and colon cancer at a later age like 70+ years . Hence all you need to do is start having a balanced fibrous rich diet consisting of fibres, vegetables and fruits. Eat more of home cooked hygienic food.
Barium Xray is an investigation which helps rule out cancer of large intestines. Other investigations like sigmoidoscopy and colonoscopy are also helpful.
As per what i feel you dont have any problems as your blood counts , urine and ultrasound are normal.It is more probably due to accumulation of gas that you are having problem.
However, for reassurance you can consult your doctor regarding the Barium Xray, sigmoidoscopy and colonoscopy.

You can apply a pain reliever ointment containing diclofenac gel like volini gel (available in india).
About your diet- it should consist of fibres like whole wheat, oats along with fruits like guava, papayas(good for digestion), citus fruits and lots f green leafy vegetables. Restrict having raw salads only at home and not outside cause the source of such salads is unknown. Take steamed vegetables with soups instead of raw vegetables.
Traditional home cooked food which is not very spicy is the best for digestive system. Restrict outside food to only once a week.
