To recapitulate: Male/46 - pain right lower back - right testicle and vomiting ...
Please give additional information for me to assist you better:
- Since when is the pain?
- Any urinary symptoms like burning urination, frequency, fever, chills or any others?
- Any swelling in the right side of scrotum, tenderness, redness?

As per the short history the reasons looks to be related to the right testicle causing referred pain in the right lower back and reflex vomiting or urinary tract infection with or without ureteric stone.

For this you need the following:
Clinical evaluation and examination by your Doctor, preferably by Urologist.
Tests of urine: routine, microscopy, culture and sensitivity.
Tests of blood.
Ultrasonography of the abdomen and scrotum with color doppler mode.

This will give you the correct diagnosis and a plan for further management.
Till then take plenty of water orally.
Scrotal support.
Rest.
