urinalysis : negative , urine culture: no growth( negative) , chlamydia: negative , gonorrhea: negative,
PSA : OK (low), syphilis: negative HIV:negative

As an Urologist,i must assure you, STD is ruled out.
1. For the rectal pain, you should have a direct per rectal examination done.
If possible, get it done by a competent surgeon.
2. The low back pain, may or may not not be, related to it.
This will require an ultrasound scan of the abdomen.
This could give us an idea,about a cause for both,rectal pain and low back pain.
Do you have constipation.?
If you start exercising for about an hour daily, it'll help relieve the back pain.
3. For ED (erectile dysfunction), you need to have the following tests done.:
Blood routine, sugar and LFT.
If you're a smoker, it helps to reduce smoking to minimal.
The same applies to alcohol intake.
4. Testicular pain, if recurrent, will need ultrasound doppler scan of the scrotum.
According to these reports, an Urologist can advise further treatment.

STD panel tests, are a group of basic screening tests.

STD negative, is reports of Gonorrhoea,Chlamydia,Syphillis, and HIV being negative.
VDRL and HBsAg also, should be negative for complete screening.
Doing it, is advisable for your safety,if you've had sex, with more than one partner.
Prostatitis is a possibility, which i can opine, only after seeing the reports of tests advised.

Your symptoms are certainly not of genital herpes.
HPV(human papilloma virus), is to considered in people with warty growths around tip of penis,or around it. You don't have anything resembling that.
Sexual contact with untreated partner can cause it.
As she's already been treated, you don't need to consider it for the time being.

If you're having regular anal sex, with an infected partner,then you need to have a rectal examination done, by a surgeon, to look for any anal lesions.
