1)How long you are having this problem?
2)Is the swelling constant always, or is it increasing on prolonged standing and decreasing on lying down?
3)Do you any associated symptoms like intermittent fever, vague testicular discomfort, burning sensation on urination, increased frequency of urination etc?
4)What about the consistency of the swelling when you feel? Is it soft, firm, or hard?

three weeks ago, after sex. I had a throbbing pain in my testis. three days later the pain had gone. and I started feeling so hot down all around my scrotal sack. I scratch a lot and it gives a burning feeling. the fifth night I reached down to my testis and I felt the lump. since the the pain comes periodically but not much. it comes after ejaculation.

From the history, it seems more like epididymo-orchitis, though the presentation is not typical. We have to do a local examination to differentiate from a varicocele and other less likely causes like cystic swellings.
If confirmed, epididymo-orchitis can be treated with antibiotics and anti inflammatory agents for a few days.
So it will be fine if you can see your doctor for evaluation. After examination, you may be asked for a few investigations like urine routine & USG scrotum.
