Suggest Treatment For Lymph Nodes In Neck For Children

Question: Hi I,
I am XXXXX..
My son is 5 years old. Diagonised (ultrasound)with multiple bilateral cervical & intraparotoid lymphadenopathy(Neck) and largest nodes shows internal vascularity.Largest node on right is 31mm and left side is 28mm.
Abdomen- umbilical quadrant shows enlarged mesenteric nodes of 12mm.
Blood test shows normal. only LDH-IFCC test is 244. Mantoux test observed 0 mm & negative. immune assay(TB quantiferon), EIA- negative-.>0.1.
Having no fever for last 3 months. Only cold and having adenoid.Chest X-ray shows slightly prominent broncho vascular markings.
Doctor has given Claribid for 15 days and given for almost 12 days. Nodes size are decreasing now. Actually what has happened to him? For adenoid Doctor given Metasray and now he is breathing in nose & sleeping well.
please help me. I am in Chennai.

Its quite common to see children in age group of 5 years presenting with lymph nodes in neck. In majority of the cases the lymph node enlargement are due to non specific enlargement, subcinical seborrheic dermatitis of scalp me also be contributing factor. Other possible cause of both side lymph node enlargement are
1. Tuberculosis
2. Lymphoma
Tuberculosis has been alomost ruled out by negative Mantoux test and negative TB EIA test. Almost possibility of Lymphoma is less likely as there is no significant elevation of LDH. As you have mentioned that lymph nodes have started decreasing after initiation of antibiotic therapy, so further making it more likely o be non specific enlargement of Lymph node.
Majority of Pediatrician/Pediatric Surgeon rule out TB and Lymphoma and put the children on antibiotic. If there is regression in size and gradual disappearance then no further treatment is required.
Apart from spray, I would suggest the following for adenoid enlargement, which is also quite commonly seen around age of 5 years. By 6-7 years of age adenoids are also known to decrease in size. It occurs due to Lymphoid tissue over proliferation. Following are further suggestions
1. Steam Inhalation: 15 minutes each session twice a day preferably using electric steamer will be helpful
2. Decoction: To a cup of milk+10Tulsi leaves+2 black pepper+1 spoon Turmeric+ 2 gm of fresh ginger+ 2 spoon sugar. Bring it to boil. Filter it and give it warm to baby to drink. Continue giving it once a day. It dramatically helps children with enlarged adenoids.

There is absolutely no cause for concern regarding prominent bronchovesicular markings. Quite common during Upper respiratory infection.
Lymphadenopathy will definitely not affect growth.
