Suggest Treatment For Major Depression

Question: I am a long term patient of major depression. I am also an educated consumer. I am an RN and psychiatric social worker. The care and creativity of psychiatry seems to have evaporated. I need help desperately. Effexor worked for a few days and then I started cancer therapy. I have severe cranial/facial hyphydrosis and no one seems to care or be curious enough to help me. Tis has taken over my life the past 4 years I am SOB as well and can't stay energixed. I probably need off the drug but where do I go next.I am sad, low energy, weight gain,empty, hopeless helpless, unlovable. I suffer from very prolong post partum depressions in child bearing years as well as terrible pms and short luteal phase. I was and probably still am a hormone mess.

the sweating is so sever I get exhausted walking into a store. I am so sob I feel like I cannot do activities of daily living. I have passed echocardiogram and pulmonary function tests. I have lost a zest for life. My brother was a brilliant physisit. He jump from a 13 storey hotel 25 years ago . Our genetic pool sucks. My mother died of brest cancer and drank herself to death. My dad was also depressed with severe PTSD rom WW11 and finally died from it by my age 16. I have 3 very successful adult happy children. They feel used up by my depression and do not want to deal with it.

I have gone through your question in detail and I can understand what you are going through.
From what you have stated it is quite clear that you are having a depressive episode. Depression can occur with regular stress and it can also occur biologically without any stress as well. You need proper treatment for the same. There are various effective therapies for depression like fluoxetine, venlafaxine, bupropion, mirtazapine or escitalopram and they are all very effective and mostly safe. In your case venlafaxine has been tried and it has not been effective for you. In that case, the other anti-depressants should be tried. You may also require some benzodiazepines like clonazepam or lorazepam for a short period like 2-3 weeks to reduce the anxiety and help with your sleep.
If you are averse to the drug therapies then there is also an option of cognitive behavioural therapy. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.
