Question: I have type 2 diabetes (not typical presentation I've been told - 51 year old female, 5'5", 116 pounds) and have been controlling it with 15 dosage of Lantus Solostar at night before bed (fasting blood sugar was always highest; readings during the day close to normal - exercise and diet controlled). I've had more than a few episodes recently of readings in the mid to high 200s during the day, 2 hours after eating, & feeling a general malaise and fatigue (but no real specific other symptoms), but feeling unwell enough not to drive and go to work. What might account for this?

Madam, greetings from HCM. I have read your question. As I see that your on long acting insulin. That's good. Regarding your symptoms I want you to check your thyroid hormone levels, FT3, FT4, TSH, HBA1C, microalbuminuria, lipid profile. Later if required we need to adjust the dose. Thyroid hormone levels abnormalities could lead to these symptoms too.
You need to exercise. At least walk for an hour a day . take more of green leafy vegetables, oats, millets,
Avoid carbohydrates and take more of proteins.
Boiled dish using veggies, garlic, turmeric, tamarind, sprouts, or in the form of soup , boiled fish
All helps.
Lifestyle change is the key to the management of diabetes.
Based in this you make your own diet chart and record the fasting and post prandial glucose levels for a month regularly.
