default
Posted on Thu, 8 Sep 2016
Question: He was under mental confusion. Which is getting better now.
Not completely but he is calm and just enough clear thinking.
Two problems I see are -

He don't remember few things sometimes. Like he was thinking rakshabandhan is not holiday and not able to recall if it is always public holiday in schools.

Second problem , he is not blinking his eyes.
Sometimes when he blink he does not blink completely.
Although when we remind him he is able to blink properly.

Please suggest.

Brief Answer:
Please do following

Detailed Answer:
Dear XXXXXXX
Thanks again for posting at Healthcaremagic.

I am glad to know that he started showing recovery and have a clam and composed attitude these days.

Although the problems are part of the same wernicke- korsakoff syndrome but I feel following can be done for some speedy recovery.

Since he is now calm please try to engage him in some or other activities. It may be a simple assistance in house hold matters to reading a newspaper, watching a TV show, playing cards with peers or anything which makes him engaged. earlier it was not possible for us to engage in one or other activity due to his rigidness and argumentative behavior but this can be employed in current state.

This is most important as we do not have any medication to recover his memory but by making the remaining memory in use we can preserve what is left and recover fast least some of the lost memories.

For the second problem I will request you to record the number of blinks per minute and revert back to me with details. Please do it when your father is not aware of it on two or more occasions.

This will help us to determine the likely cause of the problem.

I hope this helps you.
Thanks and regards.

Brief Answer:
I think we need to evaluate him for parkinson disease

Detailed Answer:
Dear XXXXXXX
Thanks for reverting back to me.

A complete absence of blinking for 10 minutes duration is indicative of problem in his basal ganglia which control movements of our body. A severe degeneration in that part of brain can cause parkinson disease. This is characterized by tremor (mostly where at rest), difficulty in moving, mask like face, decreased blinking, decreased thinking, stooped posture among others.

Although decreased blinking itself is completely harmless but I think you need to discuss possibility of parkinson disease and depression to your doctor on next visit. These are the two condition which most often cause decreased blinking.

I hope this assists you further.
Thanks and regards.

Thanks for your reply.

I also observed some good things like-

He is catching my mistakes. He is analysing what I am saying and pointing out things like- "since you said that thing means you haven't done this." It was XXXXXXX to me.

I am not observing things which were present earlier during his confusion. If he is doing something then he is doing it confidently .

But most of the time he is staying on bed, silent , responding little to our questions.

May be feeling weakness and not interested in other things.

So is there a need for a change in his medicines like oliza and clonazepam since he dose not seem to have confusion.?

Brief Answer:
I think modafinil can be used for amotivation

Detailed Answer:
Dear XXXXXXX
Thanks for reverting back to me with updates.

It is pleasant to know that he started showing analysis capacity which is good sign of recovery. Many of the patients with recovery of such higher mental functions assume normal responsibility after a period of time. Although we are not sure but we can hope best on basis of this information.

Regarding medications I feel both clonazepam and oliza can be used with current dose as he is showing recovery and no side effects are evident. I also feel that for motivation and apathy option of modafinil can be considered.

I need to make some clarification about eye blinking. Oliza is known to cause parkinson disease like side effects and it is possible that decreased blinking is side effect of Olanzapine.

I hope this assists you further.
Thanks and regards.
