We had a discussion 2 days ago on same topic- in part.
Read your history of a silicon implant in the cheek, which had be be removed for the reaction it created. This was opened and removed in the office.
Silicon is radio-opaque meaning it is seen on X-ray / CT scan.
It is possible that the a very small part of silicon sheet might have remained in the layer when it was inserted. These three particles may be seen on the CT scan as metallic.Since you have got a reaction with silicon, it is possible that the remaining very small parts must have caused the reaction to give a picture as seen in the CT scan. Edema (swelling) in this part and particularly after an associated infection might have caused the present situation as seen clinically and on CT scan.
Granuloma in simple language is cornification of the tissues. This is body's natural response to any foreign body to encase it, ward it off from rest of the body so that it is segregated to cause least possible side effect. This is picked up as hypodense areas in the CT scan, in which the 3 mm metalic (silicon in your case) foreign bodies are seen.
Because of the reaction, hematoma or seroma formed. This is the richest medium in which the bacteria can grow very fast, hence you might have got the infection.
I can understand the plight you must be going through.

With the condition you are in at the moment and as you explained, I think we should concentrate on the medicinal aspect till the infection is settled.
