Suggest Treatment For Microscopic Hematuria
Please discuss benign familial hematuria with urologist and then proceed
Detailed Answer:
Hi XXXX,
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. Since the other blood reports are fine and CT scan results are clear, therefore the next step in the evaluation of microscopic hematuria is cystoscopy which is to look for any cause in lower urinary tract and urinary bladder.
2. The CT scan is primarily to evaluate any major causes for microscopic hematuria including kidney infection and tumor.
3. It is important that benign familial hematuria is discussed with your doctor. This condition is called thin glomerular basement membrane disease and the changes occur in the glomerular basement membrane in kidney leading to microscopic or visible hematuria. Since it is a benign condition which does not affect the overall health of the patient, it is not investigated in detail if there is sufficient proof that the patient has this condition.
4. The cystoscopy is used to evaluate the lining of the urinary bladder and to look for any focus for the microscopic hematuria.
5. I suggest you discuss the possibility of benign familial hematuria with the urologist and then proceed with a cystoscopy, if clinically indicated.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.
Regards,
Dr.Vivek

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
1. Cystoscopy is one of the last tests to evaluate microscopic hematuria. This test completes the test menu and makes sure that the important causes have been investigated.
2. In the earlier reply, I have written that your urologist should know that benign familial hematuria runs in your family and then proceed with cystoscopy. If you have not discussed this point then please inform him at your next visit.
3. The urologist does not want to leave any stone unturned when investigating causes for your hematuria. Therefore having a normal cystoscopy will confirm that he is not missing anything and you are safe.
Hope your query is answered.
Do write back if you have any doubts.
Regards,
Dr.Vivek
