Brief Answer:
Please take treatment and do ultrasound scan after 8 weeks

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. The uterus is slightly enlarged and this is usual in some women. It is not a serious concern.

2. There is a small 22 x 12 mm complex cystic structure in the right ovary and this is likely to be a hemorrhagic cyst in ovary, as per the doctor who did the scan. This is a common condition and happens when there is bleeding in to a cyst in the ovary, the symptoms include sudden mild abdominal pain. In most patients this kind of cyst will resolve completely and if you do another scan in September first week then most likely this hemorrhagic cyst will clear.

3. The pouch of XXXXXXX is the area behind the uterus and mild fluid might be found due to pelvic inflammation or in mid cycle due to ovulation. Please discuss this point with your gynecologist to confirm if you are having any pelvic inflammatory disease by clinical examination. If pelvic inflammatory disease is confirmed by clinical examination and your symptoms then you might have to take a short course of antibiotics like ceftriaxone or doxycycline or metronidazole as recommended by your doctor.

4.For acute spasm and pain in abdomen, your doctor might give you mefenamic acid or dicyclomine tablet for pain relief.

5. In conclusion, your symptoms and scan findings have to be matched with clinical examination by the gynecologist and a follow up ultrasound scan is suggested after in September first week to know if the hemorrhagic cyst disappears.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Thank you very much Doctor for prompt reply,
One more clarification. Why this is happened, is it because of any food habit or it is common for women. Any additional care to be taken,, the prescription given by our Doctor is exactly match with your recommendation. Our doctor advised to continue the tablet for a period of 14 days

1. Flagyl 400 - 2/day
2. Doxycyline and lactic acid Bacilcy - 2/day
3. Ranitap 150 - 2/day
4. Cycloofam MF - 2/day for pain (3 days only)

as of now we have completed the prescription for 10 days and still mild abdonimal pain continue.
Any further recommendation. Request your advice please

Brief Answer:
Please find details below

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

1. Your doctor is treating a suspected infection in your pelvis. Some times the infection is not completely treated in 10 days so taking medicine flagyl, doxycycline, ranitap (?ranitas) and lacto bacilli for 14 days is better. Ranitas is to prevent gastritis and is to be taken 30 minutes before food.

2. You might take cyclopam MF for 3 days more to reduce pain.

3. It is early now to do another ultrasound scan and please wait for 8 weeks before doing another scan.

4. Your gynecologist might have told you that maintaining genital hygiene is important to prevent pelvic infections.

5. The hemorrhagic cyst is not due to any external cause. It happens on its own and will resolve with time. If the cyst persists or grow larger in the next scan and you have severe pain during periods then it is probably an endometrial cyst.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
