Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/54 - few weeks - lumpy area to left outer quadrant - night sweats - constipation - light headedness - pain following main meal - while sitting at desk for a long period - considered twisted bowel? - not visited a Doctor - tried laxatives, exercises and increased fluid intake ...
Noted the history well.
My thoughts as per the history you have provided:
- This can not be a twisted bowel as it gives severe pain, vomiting, bloating total constipation.
- The possibilities are:
Colitis with inflammation at splenic flexture, a part of the colon at the left upper quadrant.
Stricture or mass at the same position.
This can be diagnosed by the following way:
Clinical evaluation by a Doctor, preferably a General Surgeon.
Tests of blood: routine including CEA
Tests of stool and urine.
X-ray in the standing position.
Ultrasound of the abdomen.
CT scan may be needed along with colonoscopy and biopsy if mass or stricture is suspected.
If the laxative are not working and there is impacted fecolith formation, Saline+ soap enema may help to evacuate properly
All the above things will lead to the most probable diagnosis and gives the plan for further management.
I hope this answer helps you an early diagnosis and proper timely management.
