Question: I observed a wound on opening of my anus and rash type wound in entire crack line of hip too. I have itchy feeling in it, sometime low intensity and some time more intensity itch. I did not observe blood on my underwear. Picture attached. what could be the possible problem and what could be possible remedy ?

I have gone through your query and the pictures.
Based on the picture I feel it is pile. But A detailed physical examination is essential to ascertain the diagnosis and to plan a specific management.
I would like to know since when you are facing this problem?
Do you have history of constipation?
Is there any blood in the stool?

Meanwhile avoid constipation by taking laxatives like Psyllium husk or liquid paraffin available as over the counters at pharmacies. Also avoid fatty and spicy foods which may aggravate the problem. You should eat high fiber diet, drink lot of fluids and take fiber supplements. Apply local anesthetic gel like 2%Xylocaine before defecation and before bed time.
Sitz bath: Take warm water in a tub, add 1 spoon of salt and cover your anal area, sit for 15-20 minutes. you need to do two times a day.After bowel movement and before bed time.
It will be effectively relieve you symptoms.

- which specific expert e.g. Physician, Dermatologist, Urologist etc should I consult for detailed physical examination ?
- I have this problem of itching since last two months approx but never knew its wound till yesterday when I took picture from XXXXXXX phone.
- I had observed blood in stool but it wasn't regular and most of the time I am not able to observe as I use western style comode.
- I feel sometime constipation like situation when I have to apply pressure for defecation but again not regularly.
- what should I do for healing of this wound, I am scared of it since I saw yesterday.
- I feel no pain while sitting or any time in day except if I do itching harshly.
- could you please recommend specific name of the medicine (gel) so that I can take it from medical store.
- I am attaching one more pic with little different angle, it shows some rash type sign towards testicles.

Looking forward for advise.

Based on the information it is more likely to be pile.
