
Suggest Treatment For Oral Herpetic Infection

Question: AF 150 I took for Skin Infection at Scrotum area, I faced burning in Stomach for the first day night. Then 2 day later, pain in bottom lip and now swollen and cracked and full of mouth ulcers !!
I visited my doctor, he says it is not because of the tablet. I'm damn sure it is due to it. Or any other reason , the chance may be Betadine Gargle, which i have tried for three times.
I stopped now all the tablets and applying ointment. I just taking some ayurveda for mouth ulcers control. Am i doing right or any emergency measure I have to take?

I have seen the attached pictures.
I do not relate the oral lesions to any drug reaction . I rather think the oral ulcers are due to herpetic gingivostomatitis which is basically a herpes infection of the oral cavity.
I would suggest you to see a dermatologist soon since direct oral examination is essential for confirmation of the diagnosis . You might have to be started on oral antivirals for the same then.

I do not relate the oral lesions to any drug reaction . I rather think the oral ulcers are due to herpetic gingivostomatitis which is basically a herpes infection of the oral cavity.
I would suggest you to see a dermatologist soon since direct oral examination is essential for confirmation of the diagnosis . You might have to be started on oral antivirals for the same then.

How it will happen suddenly in just 2 days.. so much of ulcers... It was perfectly right before and it happened after the consumption of AF 150 tablet. My lip got hardened and itching.. then after 3 days got this ulcer just in a night time.. I feel this is reactive to the medication.. Fluconazole is an antifungal drug and is a quite safe drug and is very commonly used for various skin problems.
I do not completely rule out the possibility of a drug reaction but I really don't think that your current problem is due to the intake of any drug.
The clinical appearance of the mouth ulcers is more like small multiple grouped ulcers on one side.
Such type of ulcers are seen in herpes oral infections. But for confirmation of diagnosis , it is essential to examine the patient directly and have a close look at the type of ulcers. That is why I suggested that you should see a nearby dermatologist for confirmation of the diagnosis .
In case you are doubtful about fluconazole being the offending agent, then you should surely avoid it's use now as well as in future too. But right now, I do not relate the ulcers to the intake of this medicine .

Checkout the skin infection picture uploaded. Whether AF-150 is required for this kind of skin infection?
I'm managing my mouth ulcer for a few days with neem leaves boiled water gargling. If not reducing for next 3 days. I will meet a new Dermatologist.
The final question. Are XXXXXXX doctors more self defending? An article says AF-150 side effects which is inclusive of the effects faced by me.. If those symptoms occur they advised immediately to consult the doctor. But when I said to my doctor, he didn't analyse a little even (by asking some questions) to check it is due to medication allergies.. but pointing simply it is my assumption, if I feel so quit !! Do i need to change my doctor?

You surely have a scrotal fungal infection called tinea .
If you ask me, even I would have given oral fluconazole along with a topical antifungal cream to my patient with a similar infection.
Fluconazole is the first line of treatment and the most widely used antifungal drug and is widely used by dermatologists .
I am not defending anyone but I am just giving my personal opinion .
Now as far as side effects of fluconazole are concerned, it does have many side effects , most of which are not very bothersome . It can cause oral ulcers rarely but the type of oral ulcers that occur as a side effect of fluconazole are mostly either fixed drug eruption or they can be due to lesions of XXXXXXX XXXXXXX syndrome or ten.
So the type of oral ulcers that can occur after fluconazole intake are completely different looking, according to my knowledge .
But yes, if a patient relates any symptom to the intake of any particular drug, then we cannot totally ignore his call. Stopping the intake on drug is mostly advised.
Since I suspect a herpetic cause in your case, I had advised you to see a local dermatologist .
You can however do betadine gargles at home. Although this episode is self limiting and will heal by itself in few days, but if at any time you feel that the ulcers are persisting or increasing , then you surely should not wait and see a doctor soon.
As far as the groin area is concerned, you can apply a topical antifungal cream like candid cream over the affected areas twice a day which should take care of the fungal infection .
Lastly , I would say that there is never a harm in taking a second opinion . If , due to any reason you are not satisfied with your doctor, then you have every right to consult another doctor. But in any case, it is important to trust the one you are taking treatment from.
