I am sorry for your distress but if the pain is more severe than usual and is not responding to medication then cord compression must be ruled out via an MRI. Its vital because failure to do so can affect ambulation adversely if prompt diagnosis is not made. If the MRI is clear then the pain should be managed with additional options. The spasm description of pain is common for neuropathic pains though bone pain can also have a spike and dome pattern especially if the osteoporosis is severe. Its unlikely to be dehydration as dehydration doesn't cause pain. I would recommend Gabapentin as an option. You may discuss these options with your doctor to reach a final conclusion.
