I read your question and i understand your concern. Folate levels if low should be corrected. B 12 levels if low are problematic and slight raised levels like yours of 1280 is acceptable if there is no other hematological or metabolic disorder or liver disease which you don't have. Result slight high can be due to the random taking as we recommend a 6 to 8 hours gap between after before doing it. also the medication you take can cause higher B12 levels results. All your work up and blood reports and CT etc are normal and that rules out any serious pathology. Not to worry about. You just need folate supplements.
Foods like green leafy vegetables , beans, lentils, spinach, asparagus, lettuce, avocado, broccoli, oranges and white bread etc are rich sources of folate and should be prefered in diet.
Nut shell all your work up is normal. take folate supplements, don't discard other vitamin supplements. Use them in recommended dosages allowance for a day. Rest continue your medications you have been using. Nothing is worrisome here .

also I have lost like 18 pounds since may and not trying to. I have the lap band and I haven't had a fill in 2 years. I have a large ventral hernia, need an abdominoplasty and had a hiatal hernia repaired in 2010 when I had the lap band. I sometimes feel like I have a large lump in my throat and I don't understand the losing weight. I don't eat as much because I don't have an appetite. My lap band dr. said to let him know on Tuesday if I wasn't better and he would refer me to a gastro. Just burn in upper chest and esphophagus and stomach. Should I let him know? And I have lost 3 pounds this week.
oh and had a normal pelvic ultra sound in july as well

Losing weight is attributed to lap band surgery . If you did not needed losing weight why did you opt for lap banding. Pace of losing weight can be adjusted though by adjusting the fill of the band. Talk to your gastroenterologist and let them have a look at filling status of the band and decide what is best for you.
Something stuck in throat feeling and chest pain with burning sensation is a sequelae of this surgery as well and is due to GAstroEsophageal Reflux Disorder. You will need more than prilosec. Eat small frequent meals, minimal spices, stimulants and sweets like chocolate etc.
Good to hear that pelvic ultrasound is normal.

of all i would expect the least from a well educated medically informed personnel like registered nurse to be that anxious for a slight decline in weight with ban in place and jumping to conclusions of GI malignancy.
I must say anxiety is kicking in the worries you should not have. It s very unlikely to have cancer and malignancy does not show this way only . That has sequelaes and systemic manifestations like lymphadenopathy , hemetemesis, melena , dysphagia etc.
Its not malignancy. its just the overfunctioned band not filled and adjusted for last two years. Just get an assessment and let them take care of it.

As i mentioned earlier it could be all possibilities but cancer is not one of them. But there is no harm in getting it out of the differentials list. Endoscopy will fix that. As far as losing weight is concerned that is due to the lap band and not the malignancy. Hernia recurrence can be ruled out by a simple examination and an ultrasound to rule out any hernia. Epigastric pain, back pain and all other abdominal discomforts are attributed to the lap band and are common sequelae also anxiety is working like petrol to increase the heat of this fire of worries which is not necessary at all.
Just calm down, relax and wait for your Gastro appointment and let them take care of it.
