Brief Answer:
Consult a General Surgeon, get proper diagnosis first.

Detailed Answer:
Hi XXXXXXX
Read and understood your problem of some problem at the anus for the last 2 years and you are taking Ayurvedic treatment.

The pain, itching, wound and increased pain after passing stool indicate that you have combined problems.

My thoughts:
It looks you are suffering from anal fissure.
Diagnosis of pile is always confirmed by the small tune passed into the anus. There is no other way that can tell whether you have piles (enlarged blood vessels-veins) or not.

So I would advise you to consult a General Surgeon:
He would fist do clinical assessment and give you a proper course of an antibiotic, Metronidazole, probiotic, laxative and painkiller with anti-inflammatory propertied. (These medicines need a valid prescription)
Once the pain is reduced, ano-proctoscopy will give you the perfect diagnosis and a plan for the treatment.
- 2 years is a long time time to suffer, dear friend; this would have been long cured if you had consulted a General Surgeon.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more if you need to or if you feel that there is a gap of communication.

Brief Answer:
Get a prescription from the local Doctor.

Detailed Answer:
Hi XXXXXXX

Why is it not possible to see a Surgeon at the moment? Please try one.

The prescription based medicines are not available without a valid prescription as the Pharmacy store will not give you; you may then please consult a nearby qualified Doctor to get a prescription of an Antibiotic, metronidazole, probiotic, Laxative in tablet or syrup form to have loose stool.
You can take painkiller like Ibuprofen.

Take Curds in fiber rich diet.
Sitz Bath: sit in a tub with warm water, helps a lot.

I hope this helps you.
Which city are you residing at the moment?
