Brief Answer:
He needs to visit ER as the hernia could stuck.

Detailed Answer:
Hello!

Thank you for the query.

Hernia is a kind of hole in the abdominal wall were intestines can get through. In some cases such intestine can stuck in a hernia and can not be pushed back to the abdominal cavity. In such case, intestine can get compressed what causes bowels obstruction or/and bowels ischemia. Such situation is life threatening and without emergent surgery can cause bowels necrosis and death.
This is the reason why hernia should be repaired by a surgeon and he should not wait with it.
Strong pain indicates that the hernia could stuck. As long as it wont be pushed back to the abdomen, the pain wont go away. 6 hours is enough time for intestine to get necrotic. So please do not wait any longer. He should visit ER as soon as possible. Emergency surgery seems necessary.
Nothing can be done at home with it.

Hope this will help. Feel free to ask further questions.
Regards.
