I have read your query and understand your concerns. I see that you are suffering from intense pain in the left testicle and the surrounding areas since last 2 months. You are also type 1 diabetic.
In my opinion, it seems like an on-going inflammation in the testis and the epididymis area called as Epididymo-orchitis. Your description of pain suggests so. Your doctor is right in prescribing you Doxycycline for a month; please complete the course. It could be prostatitis as well. I would suggest you to continue the medicine even though you don't see any improvement as of now.
There is also a possibility of unnoticed inguinal hernia. Perhaps, the CT scan missed it.
At this stage, it would be wise to do a scrotal ultrasound to look at the underlying pathology, if any. Was there a history of trauma to your scrotum, anytime during these days? Do you notice any scrotal swelling?
It does not sound like STD to me either. How are your blood sugars now? Are they in good control? Let me know the latest values.
Again, the increasing pain in testicles when leaning forward, sitting makes me think of hernia as a possibility again. Why don't you consult a skilled surgeon and get examined for inguinal hernia again?
If you still do not find any good reasons, then you would have to repeat the CT scan.
