Suggest Treatment For Pain In Intergluteal Cleft And Shooting Pain During Bowel Movement

Question: I had a sexual in counter with my male partner of 5 years (anal intercourse) about 10 days ago. A few days after I noticed pain between my buttocks and also pain when passing stool (sharp pain - i have had anal fissures before) but no blood. I have noticed a red sore between my buttocks that is very itchy and uncomfortable especially when walking. Is this an abrasion or Herpes Simplex? I live in a country in which it is hard to get STD testing.
Brief Answer:
Possibility of genital herpes
Detailed Answer:
Hello. Thank you for writing to us
I have gone through your query and I have also reviewed the Image. I can see grouped lesions/erosions with brown crusts over it and surrounding redness.
This can be genital herpes, specially considering the morphology and the fact that it happened within a few days after sexual intercourse and was also associated with symptoms like pain which were most likely prodromal symptoms of herpes.
This possibility could only be confirmed by a lab test.
Regards

Thank you Dr. I also add that I have had swelling of lymph nodes in the groin area of the affected side.
However, seeing as we are in a monogamous relationship, how is it possible that these symptoms are only showing up now and not previously if one of us has the virus.

Brief Answer:
Herpes can remain clinically silent for long periods of time
Detailed Answer:
Hi.
Clinically the image looks very much like that of genital herpes, however, herpes is a possibility which needs to be confirmed by lab tests.
Herpes can remain clinically silent for months or years and most people are not aware how and when they acquired the infection, specially if they have had different sexual partners in the past.
