Brief Answer:
very less possibility of kidney damage to show up at 3 weeks.
Detailed Answer:
Hello,
* As per my clinical experience, the possible kidney injury will not remain silent for 3 weeks period; hence its chances are very less.
I would like to answer further with following information
1. Pain - unilateral or bilateral, type, frequency, radiation, particular association with posture changes if any where
2. Associated vomiting, fever or other systemic features
3. Any medications being taken currently
4. Is urination okay or any blood, burning in urine
Awaiting for your response.

Detailed Answer:
Hi,
* The previous connection with positional changes exacerbating the pain indicates most possible issue with ligament or muscle which may be hematoma or else.
* After certain period of time the process of inflammation starts and then it becomes static irrespective of postural changes and such issue is not evident on X-ray.
* In our clinic, I recommend an MRI of the area for further in depth evaluation and definite plan of management as pain is constant and medicines are not helping to desired level.
Hope this information will clear your doubt. Welcome for further assistance.
