-- It can be a muscular pain and the swollen part it is a muscle who is tensed and contracted.
-- It can be a chain of swollen lymph nodes from another reason, so not ear problem.
-- Or it can be a thyroid problem.
Can you give me some more information about other problems that your son has?
Fever? Sore throat? History of thyroid problems? Are the antibiotics that he is using helping?

With the information you have given to me I will still stay on the answers I have given above because those are the most possible diagnoses
So I think one or the other of those diagnoses stands.
Give him Ibuprofen or another analgesic for the pain and headache.
Also, if antibiotics are working to remove the inflamatiom on the lymphnodes, continue with those until the cure it is done.
If not better in about 1 week, consider a neck ultrasound to check what type the lymohnodes are, how it is the thyroid, and if something it is noticed there he may need some blood tests.
A physical exam to a local doctor may reveal findings that we cannot evaluate from here. If he has sore throat, pus in the tonsils then most probably the lymphnodes are from it and the antibiotics will help.
