Brief Answer:
Should rule out other pathologies first...

Detailed Answer:
Hi my dear,

I am sorry for the pain you are experiencing.

I too think the pain is due to arthritis in your back (spondyloarthritis), however, if I was your caring doctor would make sure that the pain is really from arthritis. So, I'd recommend a few tests to rule out other pathologies as following:
- abdominal ultrasound (uro-genital problems)
- urine analysis (urinary tract infection)
- complete blood count (appendicitis)
- X-ray of your back and pelvis area

For the moment, I can advise only otc pain killers like ibuprofen or mephenamic acid and heating pad locally until you get the test results.

Hope it helped! Let me know if you have further queries!
