Suggest Treatment For Pain In The Perineum Area

Question: Hi Doctor
The area between the Scrotum & the anus, i.e the perineum area has always had a protrusion in the centre & has been there ever since i can remember. Off late the bulbous centre area has had a slight swelling & is causing a mild discomfiture. This is specially felt during long sitting session & during walking. When sitting i can feel the underwear or the jeans stitching pushing on to the area & hence the discomfiture. But when even which loose clothes, the protrusion can be felt significantly though this feeling wasn't there earlier, though the protrusion has always been there. I have also put on a little weight since the last few months & the clothes i wear have become a little tight. Is this a normal discomfiture due to the clothes or is there some infection that needs to be addressed. The piano is very mild or maybe I'm paying too much attention to the thing but i am a little worried. Thanks

Thanks for your query,based on the facts that you have posted it appears that you have feeling of discomfort in perineum.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have any urinary symptoms ?
2) Do you have pain in perineal region?
3) Do you have fever?
4) Did you sustain injury to perineum?

In absence of any urinary symptoms the cause of protrusion in perineum could be due to either Weak Perineal muscles or Excessive fat in perineal region.

Please consult qualified Urologist for clinical assessment ,evaluation and further treatment.

Wear loose under wares and taking pain killers like Diclofenac fir a week should help to relieve discomfort.

Dear Doctor

Many thanks for the detailed reply. To your queries, please find my detailed reply point wise as raised by you. Hope this will enable you to get a clearer picture of my predicament. The details are as follows
1. Urinary symptoms- Other than an occassional burning sensation which is felt while passing urine, no other urinary symptoms. This sendation is especially felt when i do not consume sufficient water through the day. This burning happens, say once a week the day i drink less water fluids. Other than this occassional burning sensation ive had no other symptoms. I got a urine analysis done in Feb2015 which came out ok. Shall try to send you the report also.
2. Pain in the perineal area. I do not have a pain in the perineum area as such. Like mentioned before there is a small protrusion in the perinem area behind the scrotum & towards the anus in the centre. This has been there since i can remember & the size of a small green pea. Lately it has formed into the size of a peanut which causes mild pain when rubbing with my garments but when with loose clothes, its perfectly fine. Secondly when feeling it also, it give a slight discomfiture feeling & alsi when walking. Otherwise no pain that is not bereable. Infact unless touching, walking or grazing against clithes, i do not feel anything.
3. I have not had any fever till now since this development of the protrusion along with a mild discomfort.
4. I do not remeber having faced any injuries to the perineum. Not that i can remeber. Only problem is with the undergarments & jeans which run against the area on a regular basis causing discomfort & pain
Hope the details given above are sufficient to lead you to a conclusion. I will visit a doctor at my area. Should i visit a specalist, say a Urologist or do i need to visit a dermatologist or for that matter any field that specalizes in this area. Seek your advice.

Based on your statement I think the lump with long duration is likely to be either a Sebaceous Cyst or a soft tissue tumor Lipoma .

However precise nature of the lump can be confirmed only on clinical examination.

Please consult qualified Urologist for clinical examination.

Further treatment will depend upon the clinical finding and final diagnosis.
